My 2017 Bolt has a J1772 Combo charging receptacle. The manual says to open the bottom two holes for 220 charging. The 240V chargers listed here and elsewhere seem not to have the 'combo' extra 2 ports below. What's up here?

You are confused.

Those two ports (CCS) are for DC fast charging (DCFC), 350-400V DC, sometimes called Level-3 or ultra-fast charging.

240V AC charging is level 2, and just uses J1722. It is sometimes confusingly called fast charging by people who call DCFC ultra-fast.

The round plug has the handshake, two "hots", and a ground. The DCFC is done over the two pins located in the oval section just under the round plug.

To clarify for any other newer owners who may come across this thread, page 234 in the manual describes opening the cover for the bottom holes for DC charging (not AC, or 220v), which is ony for the DC Fast Charging described in the previous posts.

That cover is orange, due to OSHA requirements that any wiring component must have an orange insulation as a warning of the higher voltages (over 300V) just like all the orange cabling between the battery, controller, and motor in the front bay. When the first prototype Bolt EV was being road tested, many at the "gm-volt.com" site were asking if the Bolt EV had DC charging. I found one image in 2015 that showed the orange cover. I posted that image, and told the members to look for that orange cover inside the charge port when they went to see the new Bolt EV at a Chevy dealer as evidence that it did have the DC charging feature.

So if any member here wants to buy a Bolt EV or any new EV with DC charging, the SAE J1772 standard mandated that extra orange cover over the DC input.
